When Mr. Frank Acosta arrived at Creekside
Terrace, he required maximum assistance for
transfers, could not ambulate, and was
unable to don his prosthesis due to swelling.
Basic tasks like dressing, toileting, and
bathing were significantly challenging.
Now, in preparation for his discharge from
therapy, his progress is nothing short of
remarkable. Mr. Acosta completes transfers
with standby assistance—including car
transfers—and walks 200 feet using a 2-
wheeled walker. He independently dons and
doffs his prosthesis and handles grooming
on his own. He requires only minimal
assistance for bathing, dressing, and
toileting.

Mr. Acosta’s determination and the support
of his care team have been key to his
recovery. His story stands as a testament to
perseverance and the power of
rehabilitation.
